BMI ≥ 40 kg/m2 , morbidly obese. With or without coexisting medical problems and in whom bariatric surgery is not associated with excessive risks.
BMI ≥ 35 kg/m2 and 1 or more medical problems associated with obesity
BMI ≥ 30-34.9 kg/m2 with diabetes or metabolic syndrome, can be offered surgery

There is insufficient evidence to recommend surgery for glycemic control only, or for lowering of cholesterol. There is no evidence for reduction of cardiovascular risk independent of BMI.
